Here are the variety of Services we provide:

1. General Home Health Services:

The Ohio Medicaid PASSPORT 
waiver program allows seniors that require a nursing facility level of care to remain living at home, or the home of a family member, and receive care in those locations. ... Under PASSPORT care, family members can be hired to provide personal care
Private duty home care is just a sub-set of general home care. It is kind of a "Jack-of-all-trade" service where a caregiver can render services ranging from healthcare to bill paying, errands, and even transportation services.
Private Duty Nursing is a Medicaid State Plan service that provides in-home skilled nursing care to Medicaid clients of any age who require continuous nursing services beyond the Medicaid State Plan Home Health benefit.
Clients are usually seniors, people with disabilities or illnesses, or those with conditions which limit general self-care. Live-in companions, may be required to make and or serve meals, run errands, and assist with personal needs such as dressing and bathing.
Memory care is a distinct form of long-term care designed to meet the specific needs of a person with Alzheimer’s disease, dementia or other types of memory problems. This group requires 24-7 care because of documented tendencies to walk away from their dwellings.
Homemaker/personal care (HPC) refers to a variety of services, supports and supervision necessary for the health and welfare of an individual which enables the individual to live in the community. These are tasks directed at increasing the independence of the individual and at the individual's immediate environment that are necessitated by his or her physical or mental condition (includes emotional and/or behavioral). These tasks may include grooming, cleaning/maintaining a home, managing money, taking care of medical needs, and accessing the community. Without this service, alone or in combination with other waiver services, the individual would require institutionalization.
"Waiver nursing services" means services provided to an individual who requires the skills of a registered nurse or licensed practical nurse working at the direction of a registered nurse.

"Waiver nursing delegation" means activities related to the transfer of responsibility for performance of a specific nursing task from a licensed nurse authorized to perform the task to unlicensed personnel. Waiver nursing delegation has two distinct components: (1)assessment - for the purpose of determining the appropriateness of delegating nursing tasks to be performed for the individual; and (2) consultation - with an individual, a physician who ordered a delegated nursing task, or unlicensed personnel to whom the delegating nurse has delegated responsibility for a nursing task.
Shared living" means individual-specific personal care and support necessary to meet the day-to-day needs of an adult enrolled in the individual options waiver, by an adult caregiver who resides in the same home as the individual receiving the services. Shared living is provided in conjunction with residing in the home and is part of the rhythm of life that naturally occurs when people live together in the same home. 
Services provided to individuals unable to care for themselves that are furnished on a short-term (60- day) basis because of the absence of or need for relief of those persons who normally care for the individuals. Community respite shall be provided only outside of an individual’s home in a camp, recreation center, or other place where an organized community program or activity occurs. Trusted Care requires a minimum of 30 days advance notice from any client interested in a community respite service. This helps us better prepare for this client.

2. Services through Ohio Managed
    Care Organizations (MCOs):

Trusted Care provides services as a contractor for the following Ohio MCOs:
*CareSource
*Aetna
*Buckeye
*Paramount
*Molina

 

3. Adult Day Support Services:

"Adult day support" also known as Adult Daycare, means provision of regularly scheduled activities in a non-residential setting, such as assistance with acquisition, retention, or improvement of self-help, socialization, and adaptive skills that enhance the individual's social development and performance of daily community living. Adult day support shall be designed to foster the acquisition of skills, build community membership and independence, and expand personal choice. Adult day support enables the individual to attain and maintain his or her maximum potential. 
b. We provide Adult Day Support Services in 2 categories: The first is for Developmentally disabled clients (DD or DODD), and the the second (which is in a separate building) is for adults just going through the normal aging process, but with no developmental disabilities.

4. HPC Transportation Mileage and Non-
     Emergency Medical Transportation
     (NEMT) Services:

"Transportation mileage" means a transportation service offered by a provider OTHER than medical transportation available through Ohio's approved Medicaid state plan and non-medical transportation. Commonly referred to as "ATN" services, it is a transportation service for anyone with disability that does not need a wheelchair.

NEMT (Non-Emergency Medical Transportation) on the other hand is a service most commonly used by anyone with disability that requires specially built vehicles with wheelchair capabilities. Trusted Care Corp is certified to provide both ATN and NEMT services in all 88 Ohio Counties.

5. Money Management I/O

"Money management" means services that provide assistance to individuals who need support managing personal and financial affairs, including training to assist an individual to acquire, retain, or improve related skills. The services meet a continuum of individualized needs, from organizing and keeping track of financial records and health insurance documentation, to assisting with billpaying and maintaining bank accounts. Money management does not take the place of services provided by professionals in the accounting, investment, or social services fields. Money management complements the work of other professionals by facilitating the completion of the day-to-day tasks rather than determining or executing long-term plans. Money management includes a broad range of tasks determined necessary in the individual service plan.

Our Money Management/Representative Payee Services Include:

  • Bill-paying and preparing checks

  • Balancing checkbooks, reconciling bank account statements, and maintaining or organizing bank records.

  • Preparing and delivering bank account deposits.

  • Assisting the person with applying for benefits and assisting the person with maintaining eligibility for benefits.

  • Consulting or making referrals for consultation regarding available benefits.

  • Making referrals as appropriate for establishment of disability accounts.

  • Organizing tax documents and other paperwork.

  • Negotiating with creditors.

  • Deciphering medical insurance papers and verifying proper processing of claims.

  • Providing general organization assistance.

  • Providing referrals to legal, tax, and investment professionals.

  • Notarizing documents.

  • Providing assistance associated with financial tasks when a person relocates.

  • Acting as power-of-attorney or authorized representative when designated by the person, and being the appointed payee.

6. Individual Employment Support I/O

"Individual Employment Support"  (IES) means individualized support for an individual to maintain competitive integrated employment. Activities that constitute individual employment support include but are not limited to: Job Coaching, Training in assistive or other technology utilized by the individual while on the job, other services not specifically related to job skill training that enable the individual to be successful in integrating into the job setting, personal care and assistance.
  • We use a variety of social-media platforms to connect with people we serve, matching their programming with the platform that makes the most sense to their needs. 
  • We also deliver in a variety of ways, such as remotely checking in with the person outside of their workday to review their work successes, challenges, and offer any interventions.
